If you have qualified to receive a $100.00 dining dough certificate code and have not activated your first $50.00 dining dough certificate code, please follow the instructions below:
Step 1. Fill out the form below to activate your first $50.00 dining dough certificate code.
Step 2. Fill out the form below a second time with the same information you supplied to activate the first
$50.00 dining dough code. You will then be able to activate your $100.00 dining dough certificate code.
